As a convenience for our patients, we have all forms necessary for your Initial visit available to you. Additionally, please take the time to fill out these forms if you are a returning patient but have not been seen in more than 12 months or are being seen for a new condition. Please download the forms from the link below. Please print out, fill in and bring in with you to your appointment.

Also, if you should have any questions prior to your appointment, please feel free to contact us. You can simply click on the "Contact Us" link and fill out the appropriate boxes. You will receive a response with in 24 hours or less. Additionally, you may phone our office at 559-584-3000 to make an appointment for Private Insurance and Workers' Compensation referrals. Our Medi-CAL patients can choose from three Community Care Clinic locations to see our specialty trained Physician or Physicians Assistants. The numbers and addresses for these clinics are listed to the right of the page.
